Secure online referral
Use our HIPAA-appropriate intake portal (hosted with our workflow tools) to submit minimal referral details. NPI lookup may be used later to prefill public provider data—it confirms identity for convenience, not authentication by itself.
What you can expect
We contact the patient directly to schedule. After evaluation, your office receives concise notes and a headache-focused care plan. Reach the team anytime for care coordination regarding shared patients—questions route to the headache provider or intake staff.
This workflow is not for emergencies. For thunderclap headache, new neurologic deficits, or other red flags, send patients to emergency care.
How to refer
Fax referral notes to 360-282-1043. We’ll contact the patient directly to schedule and will send visit notes and a clear plan back to your office promptly.
Care coordination: referring clinicians can reach the clinic directly for time-sensitive questions about a shared patient’s headache plan and next steps. If you’d like to discuss a case, reach out and we’ll connect you with the headache provider or care team promptly.
To protect patient privacy, please do not text protected health information to the patient-facing clinic line. For urgent concerns, direct patients to emergency care.
